We're excited you've decided to register your youngster for one of SYA's sports programs, and can't wait to work with them. We promise to do everything in our power to make their experience enjoyable and rewarding.

By clicking the appropriate link below, you will be presented with the appropriate registration form for the chosen sport. If you choose the Word version of the document, once the document is opened on your computer, simply complete the form by filling in the appropriate information, print it, sign it as necessary, and send it to the address printed on the form. If you choose the Acrobat version, simply print the form, complete it, and send it to the address printed on the form. Please be sure to include the necessary payment with the registration form.

If you prefer, you can complete the form and submit it in person during the appropriate registration period, at the time and location specified on the "Home" page of this web site.
